OceanGate’s Titan submarine broke up on its journey to the Titantic wreckage due to poor engineering and multiple testing failures on the ship, according to an official report.
The Titan exploded in June 2023, killing all five passengers on board, including the CEO of OceanGate.
The US National Transportation Safety Board (NTSB) found that the engineering process behind the vehicle was “inadequate”, resulting in errors that meant it did not meet strength and durability requirements.
The NTSB said that because the firm had not adequately tested the Titan, it did not know its actual power. It also did not know it was damaged and should have been taken out of service before its final voyage.
The Titan disappeared in the North Atlantic while attempting to dive to the wreck of the Titanic, which lies about 372 miles off St. John’s in Newfoundland and Labrador, Canada.
In August, the US Coast Guard released a damning report on the implosion that found the incident “preventable” and criticized OceanGate’s “critically flawed” safety practices.
Stockton Rush, CEO of OceanGate, piloted the Titan on its final voyage.
The passengers, who paid $25,000 each to take part in the dive, were deep-sea explorer Paul-Henri Narjolet, Shahzada Daoud and his 19-year-old son Suleman Daoud and Hamish Harding.
